Talk to a good professional piercer. When I got mine, they put some lidocaine on inside and out and it did hurt some but not as bad. I had them do it twice before the actual piercing was done.

If someone claims to have a high tolerance for pain and also claims to have almost passed out after having their lip pierced, they're lying about the pain tolerance (or they got pierced right through their whole face which I highly doubt)

 

I have had my lips pierced several times in a variety of different places, I can assure you that having your lips pierced by a professional and experienced body piercer is an altogether pleasant experience and pain-wise the sensation is not much worse to having your ears pierced. You can to an extent figure out how much it will hurt by quickly and firmly pinching your lip in the area you want it pierced with your fingernails. 

It is done very quickly and it only stings a bit, I'd say the most painful thing about getting piercings is if you knock or catch them while they're still healing. 

 

Of course it's going to hurt a bit, you're having a needle put through your flesh. Haven't you had an injection before? If you want the piercing bad enough, pain shouldn't factor in your decision to get one, as i said it stings for about a second, and after two weeks most of the healing is done (although you shouldn't change the jewelry until at least 6 weeks after)

They use numming spray\cream. it isnt 100% effective tho. my broter got his done and he couldn't talk for an hour.
